Gemcitabine-paclitaxel approved as first-line metastatic breast cancer therapy USA

The FDA has approved the combination of gemcitabine hydrochloride (Gemzar—Lilly) and paclitaxel (Taxol—Bristol-Myers Squibb and generics) as first-line treatment of women with metastatic breast cancer after failure of prior anthracycline-containing adjuvant chemotherapy unless anthracyclines are contraindicated for the patient.

In the pivotal clinical trial, median survival for 267 patients receiving combination therapy was 18.5 months, compared with 15.8 months for the 262 patients receiving paclitaxel alone. Also, average time to disease progression increased for patients on the gemcitabine–paclitaxel regimen (5.2 months versus 2.9 months), and response rates—defined as decreased pain, decreased analgesic use, and improved quality of life—were higher for patients on combination therapy.

Gemcitabine alone is also indicated for treatment of locally advanced or metastatic pancreatic cancer and, in combination with cisplatin, for treatment of locally advanced or metastatic non–small-cell lung cancer. Paclitaxel has previously approved indications, either alone or in combination with other chemotherapy agents, for treatment of ovarian, node-positive and metastatic breast, and non–small-cell lung cancer, as well as AIDS-related Kaposi’s sarcoma.
